Both agent and editor pinged a certain chapter as being a problem point, for the same reasons. On re-reading that chapter I knew, on the first page, what was wrong and how it needed to be fixed.

I think I knew it the first time around, too, but was rushing to get to the next part of the story and so tried to cram important information into an AsYouKnowBob overview.
